REQUIREMENTS FOR BOARDING

  1. All dogs must be current with Distemper combination, Rabies and Bordetella vaccines. If proof is not provided vaccines will be administered at owner's expense.
  2. All animals must be free of external parasites (ex. ticks, fleas, etc.), or they will be treated at owner's expense.
  3. Brown Veterinary Hospital has my permission to do whatever is necessary should an emergency arise.
  4. If a tranquilizer is necessary for treatment or handling, Brown Veterinary Hospital has my permission to administer such medication.
  5. Brown Veterinary Hospital allows toys, blankets, and other personal items to be left for your pet; however, we are not responsible for any broken or lost personal items.
